The New Research on 12-step: Implications for Treatment and Public Health

This workshop reviews the latest scientific evidence on 12-step mutual-help organizations (MHOs), including Alcoholics Anonymous and related programs, and their role in promoting long-term recovery from substance use disorder (SUD). Drawing on recent systematic reviews and meta-analyses, randomized trials, longitudinal studies, and economic analyses, the session will examine mechanisms of action (e.g., social network change, recovery capital, abstinence self-efficacy), comparative effectiveness relative to other professional treatments, and cost-offset benefits. Attendees will gain an updated, evidence-based understanding of how 12-step facilitation (TSF) and community-based recovery supports can be integrated into clinical care and leveraged as scalable highly cost-effective public health interventions.

John Kelly, Ph.D., ABPP, is the Elizabeth R. Spallin Professor of Psychiatry in Addiction Medicine at Harvard Medical School. He is the Founder and Director of the Recovery Research Institute at Massachusetts General Hospital and serves as Chief of the Division of Addiction Treatment and Prevention across the Mass General Brigham Department of Psychiatry.

Dr. Kelly is a past President of the American Psychological Association Society of Addiction Psychology and current President of the American Board of Addiction Psychology. He has consulted for U.S. federal agencies, international governments, the United Nations, and the World Health Organization.

He has authored over 250 publications and contributed to the U.S. Surgeon General’s Report on Alcohol, Drugs, and Health. His work focuses on addiction treatment, recovery, behavior change, and reducing stigma.

This presentation is appropriate for: clinical professionals, psychologists, therapists, counselors, and social workers.

OBJECTIVES: After this workshop, you will be able to:

1. Evaluate the Evidence Base and summarize findings from recent randomized and quasi- experimental studies comparing 12-step participation and TSF with other psychosocial interventions for SUD.

2. Describe Mechanisms of Action and key mediators through which 12-step involvement supports recovery, including changes in social networks, coping skills, and broader elements of recovery capital.

3. Apply Findings to Practice and Policy by translating current evidence into clinical and public health strategies, including systematic linkage to MHOs, integration with treatment systems, and implications for healthcare cost reduction.
